  • Drink coffee at the start of each shift for a beginning Sanity boost.
  • Refill your coffee as soon as it’s done brewing so you have a constant supply.
  • Check the camera and CCTV feeds to spot anomalies before checking them in.
  • Give anomalies the wrong medicine to get rid of them if you accidentally let them in.
  • Treat patients who are in critical condition or who have fainted first.
  • Stand on the operating table to save time during surgeries.
  • Equip eye drops to maintain your Sanity during a death ritual.

Spotting and Dealing with Anomalies

  1. Your first shift in the game is pretty much a tutorial round and always the same. You’ll only have 2 patients, and neither of them will be anomalies. There’s no need to waste your time checking them, so just let them in and treat them.
    • Once you get to the second shift, anomalies can start appearing along with your normal patients.
  2. Anomalies come up to the reception counter just like regular patients. If the patient looks a little off or different from the other patients you treated, it’s a dead giveaway that an anomaly is trying to get in. Close the shutters so the anomaly leaves if you notice any of the following:
    • Hollow or missing eyes
    • Large eyes with a big smile
    • Mismatched eyes with sharp teeth
    • 3 eyes instead of 2
    • Twitching
    • Human teeth

  3. Use the camera at the reception counter to snap a picture of each patient to check if it reveals anything unusual. Some cursed photos can reduce your Sanity, so wait for the picture to develop and look at it on the counter instead of interacting with it. Photo anomalies include the following:[1]
    • Mismatched photo
    • Different eyes or ears
    • Unnatural facial expressions
    • Static on the image
    • Bloodshot eyes and a large smile
  4. Even if a patient and their photo appear normal, there’s still a chance they’re an anomaly. Use the CCTV cameras and look at the footage to see if the patient is hiding their real identity. The types of CCTV anomalies you’ll run into include:
    • Staring directly at the camera
    • Long, unnatural body shape
    • Open mouths and sharp teeth
    • Mismatched eyes or ears
    • Censored eyes
    • Black bodies
    • Twitching

Tricks to Maintain Sanity in Animal Hospital

  1. Coffee is the main way you can keep your Sanity levels up during the game. Right when your shift starts, head straight to the coffee machine and take a cup. Each cup of coffee has 3 uses that increase your Sanity by 5%, so take a sip right away to get a starting boost.[2]
    • Save some of your sips to use throughout your shift because you automatically lose if your Sanity drops down to 0.
  2. The coffee machine has a 180-second cooldown, so it takes a couple of minutes to brew a new batch. When you hear the machine ding, go back to the check-in office and take a cup so the next batch starts brewing right away.[3]
    • You can hear the coffee machine ding from wherever you are on the map.
    • If you need coffee before it’s available from the machine, buy it from the supply shop for $55.
  3. If you’re playing online with friends, you can’t see each other’s Sanity levels. When there’s coffee available, talk to each other to figure out who has the least amount of Sanity. Make sure they pick up the coffee from the machine so they’re able to use it and help you survive the shift longer.
  4. Barney is an NPC that comes in during your shift asking for coffee. During one of your shifts, he’ll ask if he can hide in the check-in office to avoid the cops. If you let Barney hide without telling the cops about him, he’ll give you another larger coffee machine you can use for the rest of the game.[4]
    • The second coffee machine has a 300-second cooldown, so it takes about 5 minutes for it to refill.

Treating Patients in Animal Hospital

PDF download Download Article
  1. Some patients will come to the hospital in critical condition, and you only have 60-120 seconds to treat them before they die. Check which patient has the lowest amount of time, and rush to treat them before any others. If a patient fainted after checking them in or after an anomaly attack, move them into a bed before their timer runs out.
    • If a patient comes in on fire, spray them with a fire extinguisher to put them out and treat them.
    • Patients without a timer can be treated in any order.
  2. There are a lot of rooms in the animal hospital, so don’t waste your time running around looking for your patients. When a patient goes into a room, a red line appears above the door so you know that it’s occupied.
  3. Each condition requires a unique item for treatment. While you still have to run tests on the patient to figure out what condition they have, knowing the treatment items means you can run to them fast and earn more money for being speedy. For the following conditions, use the following items:[5]
    • Dried eyes: Eye drops
    • Dehydration: IV drops
    • Bruises: Medkit
    • Fever: Thermo
    • Rash: Ointment
    • Bleeding: Bandages
    • Headache: Medicine
    • Stomach ache: Herbs
    • Low Sugar: Maple syrup
    • Flu: Cough syrup
  4. Room 8 has you performing surgery on your patients, and items spawn on tables surrounding the room. Jump on top of the operating table next to the patient and spin around to grab treatments from the other tables. This saves you valuable time because you don’t have to run around the room to collect the treatment items.
  5. Normally, you have to equip a treatment item and manually give it to a patient for their treatment. The “Give Medicine from Inventory” upgrade automatically searches your inventory and uses the required items right away without equipping them. Check the Supplies Shop frequently for this upgrade because it saves you time and lets you treat more patients during a shift.
  6. If you realize that one of your patients is actually an anomaly, just give them the wrong medicine to kill them. While you’re normally penalized when a patient dies, killing an anomaly doesn’t give you any penalties.[6]

Animal Hospital Anomaly Enemy Guide

  1. Bed Monsters hide underneath the hospital beds and can pull you or another patient underneath. Avoid the red area in front of the bed while there’s a monster around. If you have a transplant, organ, or maple syrup in your inventory, equip it while walking toward the monster. The monster will take it from you and disappear without any trouble.
    • You can also lure other monsters that chase you toward a Bed Monster, and they’ll get attacked instead of you.
  2. The Head Banger anomaly pounds their head on the check-in office’s windows and prevents you from closing the shutters. Head out to the waiting room and offer the Head Banger a sip of your coffee. After that, they’ll leave without taking away any of your Sanity.[7]
  3. Ghosts spawn when a patient dies, and they can turn invisible if you get too close to them. If you suspect a Ghost is nearby, spray it with the fire extinguisher to make it reappear. Then, use a taser or gun to take the ghost out for good.[8]
    • If a Ghost attacks you, you lose 2 Sanity every second.
  4. The Mass of Eyes is a large red blob on the ceiling with multiple eyeballs, and you lose Sanity every second you look at it. Keep your eyes on the floor and avoid looking up to stay safe. Otherwise, equip eye drops and use them on the Mass of Eyes to neutralize it.[9]
  5. Mimics (or Skinwalkers) can transform right after treatment, and they’ll attack the closest player or patient. Run away to get some distance between you, and use your taser or gun to take them out. It takes 2 shots to beat a Mimic, so make sure you have some ammo stocked up.
    • You can purposefully lead a Mimic to a patient to buy yourself more time. However, it will trigger a critical emergency and prevent you from making as much money during your shift.
    • If you don’t have a gun or taser, you can fight a Mimic with your bare hands, but they’ll attack back and make you lose Sanity.
    • As soon as you beat a Mimic, it will transform back into a patient and leave the hospital.
  6. The Stalker is a tall black figure that makes you lose sanity whenever you make direct eye contact. As soon as you spot one in the room, look away for a few seconds so it disappears.
  7. The Camera Figure appears on broken CCTV cameras, and it causes a zoom-in jump-scare if you keep looking at the footage. Exit out of the camera as fast as you can to avoid losing Sanity. Then, go find the camera in the hospital, and interact with it to repair it and remove the Camera Figure.
  8. Hiders are the same color as the wall and they stay hidden until you notice and approach them. When they start chasing you, simply run away to get some distance between you. The Hider will disappear on its own after about 5 seconds.

Other Events and Emergencies in Animal Hospital

  1. Fires can break out in a random room in the hospital, and they’ll cause patients to faint and you to lose Sanity if you walk through the flames. Use a fire extinguisher on the flames closest to you and slowly enter the room to put out the rest of the fire.[10]
    • Leave a small patch of fire in the corner of the room to prevent any other fires from spawning during the shift. It won’t spread or hurt anyone as long as it’s out of the way.
    • If a patient faints because they walked through a fire, put them into a bed before their timer runs out.
  2. Ambulance events can occur randomly during your shift and fill your hospital with patients. Patients (and anomalies) will run into any available room without checking in first, and some of them may be in critical condition.[11]
    • Prioritize helping patients in critical condition first so you don’t get penalized.
    • Double-check CCTV cameras in the rooms for potential anomalies so you don’t get attacked.
  3. If anomalies make it into the hospital during your shift, they may trigger a ritual inside the room. During the ritual, lit candles appear in the room and the patient starts levitating over their bed. Put out all the candles in the room within 40 seconds to prevent the patient from dying. Each time you put out a candle, you lose 1-3 Sanity.
    • If you equip eye drops while you’re putting out the candles, you won’t lose any Sanity.
  4. If a patient or anomaly is in Room 8, there’s a chance a purple tendril monster will come out of their body. When it happens, you have 45 seconds to complete their treatment in order to stop the monster and save the patient.[12]
    • The first items for the monster treatment in order are always scissors, transplant, scissors, and transplant. Then, it requires 3 random treatment items.
